BACKGROUND Preserflo draining devices have been recently launched to tackle intra ocular pressure. Straight forward surgical technique and use of high-tech materials are promising good results for glaucoma patients. Although the device specific design should prevent postoperative hypotony, this complication is observed in up to 10% of postoperative cases. Suture stenting has been described as an effective way to prevent over drainage. HISTORY & SIGNS We report a case of sustained postoperative hypotony which was managed with multiple stenting sutures to improve intraocular pressure control. THERAPY AND OUTCOME After failing conservative hypotony management, the decision was made to perform a surgical revision of the glaucoma implant in order to limit outflow. Two stenting sutures were inserted into the lumen of the drainage. A third suture was used to ligate the implant. This technique allows safe tailoring of drainage in the postoperative phase in this complicated case. CONCLUSION Preserflo microshunt stenting with multiple sutures offers a safe way to moderate postoperative drainage in challenging cases. To our knowledge this technic has not been presented before. |
