Abstract-Text Purpose: To report a case of successful pain management in a patient with severe neuropathic pain due to perineural orbital infiltration of a conjunctival mucoepidermoid carcinoma.

Methods: An 85-year-old female patient with severe orbital and ocular pain on the left side, which could not be adequately treated with topical and systemic pain therapy due to perineural infiltration of a locally metastasized conjunctival mucoepidermoid carcinoma. The cornea appeared completely vascularised with poor light perception in the left eye, whereas the right eye had normal ocular findings with good visual acuity. The patient had strictly refused radical and potentially curative treatment by orbital exenteration surgery or tumour excision with adjuvant chemotherapy or radiotherapy four years prior.

Results: Retrobulbar ethanol injection (ethanol 96%, 1.5ml) achieved significant pain reduction with sustained efficacy over four years, resulting in a significant improvement in quality of life.

Conclusion: Retrobulbar injection of ethanol is a promising alternative therapy for chronic orbital pain, especially in the case of perineural tumour infiltration. Due to the significant side effects, especially the expected blindness due to optic nerve atrophy, this therapeutic approach should be strictly limited to cases with a severely limited visual prognosis.
